Intestinal flora transplantation and case studies

A man in his 40s with constipation disorder – Intestinal flora: a case study

Case presentation: Male in his 40s, constipation (under observation)

summary

He had been suffering from constipation for a long time. 10 years ago, he had dental work done, and his constipation became a little better. After a while, her symptoms gradually worsened, and the constipation worsened after the earthquake. An endoscopy at another hospital diagnosed spasmodic constipation. During the exacerbation of constipation, urticaria-like symptoms, nausea, and abdominal distention were also observed, and the patient became more stressed. She had to take laxatives such as Arosen every day and was always in agony. After the transplant, I got the impression that the patient’s satisfaction increased with each transplant. From the first transplant, the patient tried to avoid using laxatives as much as possible, and by the time of the sixth transplant, she was taking medication only once a week. The patient’s abdomen, which was very hard at the time of the examination, seems to have softened considerably. We plan to instruct her to maintain her current condition as much as possible and to consider additional transplants.

Transplant Information

Transplantation period: fromNovember 21, 2019 – January 15, 2020
Number of transplants :6 transplants
Change from first transplant to end of transplant :
Six transplants have been done and additional transplants are being considered.

【1st transplantation questionnaire】
I have no particular problems with my physical condition. The condition of his stools remains the same. After transplantation, abdominal rumbling is good. No particular symptoms.

【2nd transplantation questionnaire】
I don’t feel much effect yet. About an hour and a half after the transplant, I had a watery stool. The next day I had yellowish type 6 stools, the best color I’ve had in a long time. The next day it was back to normal. The pain in my lower abdomen that I had before the transplant persists, but it is slightly less severe now. Sleep is good. No change before and after transplant.

【3rd transplantation questionnaire】
This time, I was very patient and did not take out the fluid after the transplant. I did not take laxatives the same day, and the next day I had thin stools. The next day, I had a flat stomach. At that time, my skin condition also worsened. I reduced the medication and it came out again, but it was thin and blackish. I feel a change in my body. I feel good and bad. I have a sauna every day, but I feel like I sweat more easily and stay warm longer than before. The quality of sleep also seems to have improved a little.

【4th transplantation questionnaire】
The day after the transplant, and the day after that, he had a good stool (just a little darker and thinner in color), and the day after that he did not. I stopped taking Arozen (laxative) and tried to cope as much as possible with Goofys (laxative). Sleep was fine, felt like I was in a deep sleep. Skin condition is not perfect, but better than before. No stools at all and no nausea. I don’t scratch my skin anymore. I feel that I am definitely getting better.

【5th transplantation questionnaire】
I only took the goofis three times. It is good to get out every day. The skin irritation continues because I feel taut every day. Not as much as I’d like, but I definitely have the impression it’s getting better. I also feel a little better.

【6th transplantation questionnaire】
I think the progress is good. I only use Goofys once a week. I have a residual stool feeling though it comes out every day. I feel like I am sleeping more deeply. Skin irritation still occurs, but the range of skin symptoms has narrowed and is getting better.
Changes after completion of transplant :
He is still restless during the examination, but he does not grunt as much anymore.
When returning home, he still runs to the elevator as suddenly as before, but I have the impression that his communication has become more lubricated than before, as he smiles and nods his head.
He now listens to me even when my instructions are complicated, and he can recognize me even when I call him from a distance.

Evaluation and Discussion

Transplant Evaluation :Clear improvement of symptoms
Pre- and post-transplant flora balance test changes:.The diversity of the intestinal flora is not so problematic, but the balance needs a little more adjustment. After six transplants, Prevotella decreased and Bacteroides increased. This is a good trend, as it is evidence that they have begun to metabolize fat to repair skin and other tissues.
Blood test changes:.No comment
POMS2 SCORE CHANGE:.No comment

Transplant General Comments and Discussion :
The patient said he was glad he did it. He was pleased that he had recently had a normal, brownish stool that was not dark for the first time in years. He told us at the end of the session that his sleep seemed to be improving and that he was sleeping soundly. We plan to keep her as current as possible and consider an additional transplant.
