Intestinal flora transplantation and case studies

A 6-year-old boy with autism spectrum disorder

Case presentation 6-year-old boy with autistic spectrum disorder

summary

At the time of the first blood draw, the patient was sweating and resisting strongly, and three adults held him down while performing the transplant.
He came to the hospital crying until the second transplant, but after the third transplant, he became accustomed to the procedure and was able to perform the transplant without crying.
During the transplant, it was done while distracting the patient with a video and could be done without strong restraint. During the second and third blood draws, she was relatively calm and did not cry.
The parents commented that she had a tendency to have diarrhea, but as the transplant progressed, her stools became more stable, and that she seemed to be more self-aware and active than before the transplant.

Transplant Information

Transplantation period: fromNovember 5, 2019 – December 6, 2019
Number of transplants :6 transplants
Change from first transplant to end of transplant :
6 transplants conducted

【1st transplantation questionnaire】
Crying since coming to the hospital.

【2nd transplantation questionnaire】
Came to the hospital in tears again today. No change since the last transplant. Tangible stools are being produced.
During the transplant, she was engrossed in a movie. There was some physical movement, but the transplant was completed without delay.

【3rd transplantation questionnaire】
Today, she came to the hospital without crying.
I feel that she has become more active since the last transplant (according to her mother). She has started to run without shoes at kindergarten.
She has one soft stool per 1.5 days (she used to have one per day).
During the transplantation, the patient was able to perform the transplantation by 3 persons without any problems, although there was some physical movement.

【4th transplantation questionnaire】
The patient came to the hospital today without crying. She bows in greeting.
Recently, he has been suffering from a cold, which is probably the reason he has been defecating once or twice a day.
He used to have soft stools, but now he has normal stools. It may be my imagination, but I feel calmer than before (according to my parents). In the latter half of the transplant, he was a bit physically active, trying to get up.
The director recommends taking soluble fiber and probiotics. He is on a gluten-free diet.

【5th transplantation questionnaire】
When I greeted her at the hospital in a good mood, she gave me a high-five.
She said that she did not notice any significant change from the previous transplant, but felt that she was stronger than before the transplant.
He has a bowel movement once every two days. Since he defecates in a diaper, he does not know the shape of his bowel movements, but they are not particularly soft or hard. There is no change in smell.
Before the transplant, he took off his diaper and pants by himself, and was able to do so with less body movement than the last time while watching a movie of the plastic rail.
The transplant was completed without any leakage, and the patient returned home without any signs of stomachache or bad mood.

【6th transplantation questionnaire】
When we talked to her, she nodded and smiled.
He became more interested in people and asked for his teacher’s hugs at kindergarten. His stools are also taking shape, which is a good sign. (From the mother)
During the transplant, she sometimes smiles and watches the movie “Plastic Rail. She has been fiddling with her urine pad and moving her body less than last time.

【Questionnaire at the 1st examination】
Explanation of the flora balance test and blood test.
Explanation of diet therapy. Parents seemed enthusiastic.
Explained that if additional flora transplantation is to be done, it should not be done for a long period of time.

【Questionnaire at the 2nd examination】
It will take time, but the number of bacteria controlling the immune system is increasing, so we will grow the bacteria we have put in now.
From parents
I feel like I’m doing relatively well. They are now able to follow my instructions even if they are complicated. He is now able to listen to me even when I talk to him from a distance.
Hyperactivity has not changed. When he has something to do, he doesn’t stop. His stomach is better and he no longer has to go down.

【Questionnaire at the 3rd examination】
Recently, he plays alone a lot. He eats a lot of different foods, but the amount of food he eats has increased. Eats a lot of the same foods.
From now on, his goal is to take in nutrients evenly. He takes carbohydrates from root vegetables and as much fat as possible (flaxseed oil, egoma, fish, saturated fatty acids, etc.; coconut oil and ghee are also good).
There are not many foods that are extremely difficult to eat, more so than your mother. She eats what she likes best from what is on the table. She doesn’t like fish, but eats it when served.
We will continue to monitor the situation, although it is a long shot.

Changes after completion of transplant :
He is still restless during the examination, but he does not grunt as much anymore.
When returning home, he still runs to the elevator as suddenly as before, but I have the impression that his communication has become more lubricated than before, as he smiles and nods his head.
He now listens to me even when my instructions are complicated, and he can recognize me even when I call him from a distance.

Evaluation and Discussion

His/her experience (from the questionnaire) :
I feel somewhat better (although it may be within the margin of error). I’m trying to cut back on wheat and dairy.
Transplant Evaluation :Mild improvement of symptoms
Adverse events: NoNothing
Total transplant evaluation and discussion:
Reactions to instructions improved, although there was mild improvement in the effects of the implantation. He also started to play with his friends. He has started to have solid stools, but that is about it.
The fundamental improvement of autism itself is yet to be seen.
